Child Health Outreach Program Nurse/AHP

Job Summary:
The Child Health Outreach Program Nurse/AHP is a critical role within the Child Health Outreach Program, working collaboratively with children 0 – 15 years and their families to facilitate and support children with complex chronic healthcare needs through coordination of care to optimise health outcomes.
About the Role:
The position has a strong clinical focus on outreach work, including liaison with internal and external stakeholders involved in the child and family's care. The successful candidate will have a demonstrated understanding of primary health care and developing clinical knowledge of chronic disease management and/or child health.
Key Responsibilities:
- Coordinate care for children with complex chronic healthcare needs
- Work collaboratively with children 0 – 15 years and their families
- Liaise with internal and external stakeholders involved in the child and family's care
- Develop and implement care plans to optimise health outcomes
Requirements:
- Current registration as a Registered Nurse or Aboriginal Health Practitioner with the Australian Health Practitioner Regulation Agency (AHPRA)
- Demonstrated understanding of primary health care and developing clinical knowledge of chronic disease management and/or child health
- Working knowledge of Child Health and an understanding of current practices, theories and values
- Understanding and commitment to Aboriginal health, and the philosophy and practice of Aboriginal Community Control and knowledge of current issues affecting the lives of Aboriginal People
