Job Summary

We are seeking a highly skilled Diabetes Educator to join our team at St John of God Health Care. As a Diabetes Educator, you will be responsible for providing high-quality diabetes education and clinical services to clients, as well as educating and supporting people with diabetes and their families.

Key Responsibilities
  • Provide diabetes education and clinical services to clients, including inpatients and outpatients
  • Develop and implement individualized education plans to promote euglycaemia
  • Collaborate with the dietitian to provide comprehensive care to people with diabetes
  • Notify medical practitioners of patients' condition and their skills in managing their condition
  • Review hospital newly diagnosed diabetics after discharge by telephone
  • Ensure all inpatients' needs are assessed and they are equipped with the appropriate equipment for discharge
  • Maintain accurate records of patients attended
Requirements
  • Registration with the Australian Health Practitioner Regulatory Agency (AHPRA)
  • Credentialed ADE member (or working towards)
  • Commitment to continuing education and quality healthcare
  • Leadership skills and ability to mentor and coach caregivers
