About this role

We are seeking a dedicated Support Coordinator to join our team. As a Support Coordinator, you will play a vital role in assisting individuals with mental health conditions to navigate the National Disability Insurance Scheme (NDIS), interpret their individual packages, and navigate the healthcare system.

Key responsibilities include:

  • Providing support and empowerment to individuals to make informed choices about their lives
  • Offering time-limited support to individuals over complex matters
  • Coordinating services for individuals to support them in living fulfilling and meaningful lives within the community
  • Engaging with key stakeholders to ensure supports and services are leveraged and not duplicated
  • Strengthening the ability of individuals to coordinate supports and live their best life

  • The ability to work under minimal supervision, exercise initiative, and establish objectives and outcomes

Requirements

  • A relevant bachelor degree (e.g., Social Work, Psychology, Disability, Occupational Therapy)
  • Maintain membership to a relevant professional body (e.g., AHPRA, AASW)
  • Some travel to various locations may be required
  • Current nationally recognised first aid and CPR certificates
  • A Positive Notice Working with Children clearance
  • NDIS Worker Screening Clearance
  • Completion of NDIS Worker Orientation Module
