Job Title:

Social Worker Community Health Service

We are seeking a skilled and experienced Social Worker to join our team at Caulfield Community Health Service. As a key member of our multi-disciplinary team, you will provide advanced social work services for older adult clients, including psychosocial assessment, counseling, interim case management and casework.

Key Responsibilities:
  • Provide individual and group work, advocacy, and outreach to clients and their carers.
  • Demonstrate an understanding and experience in navigating pathways within the health care system, including My Aged Care, NDIS, Commonwealth Home Support Program, Home Care Packages, and VCAT.
  • Understand issues affecting clients experiencing elder abuse or family violence and respond effectively to these issues.
  • Have a Tertiary qualification in Social Work with eligibility for membership of the Australian Association of Social Workers.
  • Demonstrate Social Work skills and experience at a Grade 2 level, and have at least three years' post-qualifying experience.
