Job Title:

Care Manager

Job Description:

The Care Manager is responsible for providing comprehensive client assessment and development of care plans to support clients at home.

The ideal candidate will have a degree qualification in a relevant field and current APHRA Registration. They will also possess highly developed assessment skills, the ability to creatively problem-solve complex care situations, and experience in community-based care and support services.

Key Responsibilities:
  • Provide holistic care skills within a Consumer Directed Care framework
  • Plan, implement, and evaluate innovative health programs designed to meet the identified needs of target group care recipients/Special Needs Groups
  • Consult, network, and liaise with relevant agencies
  • Contribute to the ongoing development of the program including participating in the development of procedures, planning and reviewing of care recipient services
Required Skills and Qualifications:
  • Degree qualified in relevant Aged Care, Community Services, Social Science, Nursing or Allied Health discipline
  • Current APHRA Registration
  • Appropriate case management and aged care experience in a similar role
