Job Opportunity

We are seeking a proactive, compassionate, and forward-thinking Care Partner to join our team and play a vital role in delivering high-quality, person-centred care across the Northern Rivers region.

This role offers the opportunity to step beyond traditional case management and into leadership in action, empowering clients to make informed choices and maintain independence.

About the Role

The successful candidate will be responsible for coordinating care services under the Support at Home framework, ensuring clients receive tailored support that aligns with their goals and needs.

Key responsibilities include:

  • Deliver person-centred case management tailored to each client's objectives and requirements
  • Prepare and review budgets with clients to ensure alignment with their care objectives
  • Manage a caseload, conducting regular assessments and reviews
  • Build and maintain strong, trusting relationships with clients and families
  • Coordinate services across allied health, nursing, and home support
Requirements

To succeed in this role, you will need:

  • Previous experience in aged care case management (HCP/CHSP preferred)
  • Background in community or aged care service coordination
  • Excellent planning, communication, and interpersonal skills
  • Ability to work independently while contributing to a multidisciplinary team
  • A valid Australian driver's licence and clean driving record
  • Eligibility to obtain a National Police Check and NDIS Worker Screening Check
