We are seeking a highly skilled and compassionate Senior Care Coordinator to join our team. As a Senior Care Coordinator, you will play a vital role in delivering high-quality, person-centered care to clients across the Northern Rivers region.
- Deliver personalized case management tailored to each client's goals and needs.
- Prepare and review care plans with clients to ensure alignment with their objectives.
- Manage a caseload of clients, conducting regular assessments and reviews.
- Build and maintain strong relationships with clients, families, and healthcare professionals.
- Coordinate services across allied health, nursing, and home support.
You will thrive in this role if you possess excellent problem-solving and advocacy skills, a genuine passion for improving the lives of older Australians, and experience in aged care case management.
Required Skills and Qualifications:
- Aged care case management experience (HCP/CHSP preferred).
- Background in community or aged care service coordination.
- Excellent planning, communication, and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- A valid Australian driver's licence and clean driving record.
- Eligibility to obtain a National Police Check and NDIS Worker Screening Check.
We offer a competitive salary package, including a base salary of $80,000, plus 12% superannuation. Additionally, you will have access to ongoing training and clinical support from an experienced team, a company vehicle for work use, and a laptop.
