Senior Care Coordinator Position

We are seeking a highly skilled and compassionate Senior Care Coordinator to join our team. As a Senior Care Coordinator, you will play a vital role in delivering high-quality, person-centered care to clients across the Northern Rivers region.

  • Deliver personalized case management tailored to each client's goals and needs.
  • Prepare and review care plans with clients to ensure alignment with their objectives.
  • Manage a caseload of clients, conducting regular assessments and reviews.
  • Build and maintain strong relationships with clients, families, and healthcare professionals.
  • Coordinate services across allied health, nursing, and home support.
About You

You will thrive in this role if you possess excellent problem-solving and advocacy skills, a genuine passion for improving the lives of older Australians, and experience in aged care case management.

Required Skills and Qualifications:

  • Aged care case management experience (HCP/CHSP preferred).
  • Background in community or aged care service coordination.
  • Excellent planning, communication, and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • A valid Australian driver's licence and clean driving record.
  • Eligibility to obtain a National Police Check and NDIS Worker Screening Check.
Benefits

We offer a competitive salary package, including a base salary of $80,000, plus 12% superannuation. Additionally, you will have access to ongoing training and clinical support from an experienced team, a company vehicle for work use, and a laptop.
