About the Role

As a Care Coordinator in our PAV Response Services, you will play a vital role in empowering clients to maintain their independence and achieve their goals. You will work directly with clients, their families, and key stakeholders to implement, monitor, and evaluate our services, ensuring a person-centered approach to care.

This is an exciting opportunity to join a dynamic team where you will be responsible for coordinating and delivering services that make a real difference in people's lives. Your key responsibilities will include:

  • Managing the PAV Response Service, ensuring client care plans and documentation are current;
  • Responding to alarms and participating in the after-hours roster;
  • Maintaining accurate records, ensuring compliance with standards, and contributing to audits and continuous improvement;
  • Upholding high care standards and contributing to quality improvement initiatives;
  • Aiding clients with daily tasks and personal hygiene needs.

About You

We are looking for an experienced and dedicated individual who possesses the following skills and qualifications:

  • Minimum Certificate III in Community Services (or equivalent), including personal care and first aid competency units;
  • Strong written, verbal, and numerical communication skills;
  • Proficiency in MS Office and Client Management software (e.g., Carelink Plus);
  • Experience in care planning, assessments, and working with older adults;
  • Solid understanding of Aged Care Reforms and their impact on service delivery;
  • Commitment to delivering high-quality customer service and building strong, positive relationships with clients and their families;
  • Ability to work independently, manage competing demands, and prioritise effectively;
  • Current Victorian driver's license and national police check;
