Job Title: Community Aged Care Coordinator

We are seeking an experienced and compassionate Community Aged Care Coordinator to join our team. This role will provide high-quality care and support to clients and customers in the aged care sector, focusing on creating care plans that cater to individual needs and objectives.

Additionally, they must hold current National Police Clearance, First Aid Certificate, and SA Driving Licence. A reliable vehicle and comprehensive vehicle insurance are also essential requirements.
