We are seeking a skilled Renal and End-of-Life Care Specialist to join our care team at Blacktown Hospital.
This is an exceptional opportunity for a compassionate and experienced social worker to make a meaningful difference in the lives of patients and their families.
The ideal candidate will possess extensive specialist knowledge in renal and palliative care social work, including advanced care planning, psychosocial aspects of caring for dying patients and their families, grief counselling, and community support for those impacted by chronic illness and increasing decline in health status.
