About the Role:

The Aged Care Assessment team is a multidisciplinary group that conducts comprehensive evaluations to determine the care needs of frail clients. Our goal is to facilitate access to appropriate services through thorough assessments involving clients, carers, and providers.

Key Responsibilities:
  • We are seeking an experienced professional to provide leadership and training to clinicians in a hospital or community setting.
  • The ideal candidate will conduct assessments and focus on social work principles and comprehensive care planning.
  • This role requires effective communication and collaboration to ensure quality, evidence-based practice.
Requirements:
  • Hold qualifications and registration with the relevant authority to practice as an Occupational Therapist, Physiotherapist, Social Worker, Registered Nurse, or other relevant profession.
  • Previous experience in Aged Care Assessment is advantageous.
