Physiotherapy Specialist

The Commonwealth Home Support Program (CHSP) is seeking a highly skilled and motivated physiotherapist to join their team of experienced professionals.
About the PositionThis exciting opportunity involves working in Community Therapy Services (CTS) with clients requiring high-quality care in the community. The ideal candidate will have extensive experience in managing patients with complex needs, particularly those over 65 years of age with multiple comorbidities and falls risk.
- Triage long stay patients from the CTS waiting list for appropriate interventions
- Conduct interdisciplinary assessments to determine client needs and develop tailored plans
- Provide expert care to meet client immediate needs and refer on to relevant services as required
The successful candidate will possess:
- Expertise in service innovation and development, leadership skills, and initiative in promoting clinical excellence
- Demonstrated capability in managing patients in the community, including centre-based and home-based therapy
- Strong interpersonal and organisational skills to collaborate effectively with multidisciplinary teams
