About the Role

We are currently seeking an experienced Care and Community Coordinator to join our NDIS Services team in the Supported Independent Living Program.

In this role, you will work closely with clients and their families to develop individual living plans, focusing on building independence and community engagement.

Key responsibilities will include:

  • Developing and implementing person-centred plans;
  • Coordinating respite and recreational activities;
  • Providing transportation to/from appointments and day programs;
  • Assisting with cleaning and domestic tasks;
  • Preparing meals;
  • Shopping for groceries.

We require someone with high levels of enthusiasm, initiative, and a positive attitude, as well as excellent communication skills.

A NSW Driver's License and comprehensively insured vehicle are also essential for this role.
