Aboriginal And Torres Strait Islander Advanced Health Worker - First 2000 Days Program

We are seeking an experienced Aboriginal and Torres Strait Islander person to join our team as an Advanced Health Worker in the First 2000 Days Program.

This role involves working closely with families, advocating for their needs and providing culturally safe and compassionate care. You will coordinate case plans, conduct home visits and link families with vital services in areas such as maternal and child health, chronic disease and health promotion.

As a key member of our multidisciplinary team, you will use your strong communication and interpersonal skills to collaborate effectively with others, mentoring and sharing your cultural insight.

You will have the freedom to work independently, influencing real change in individual lives and across the wider health system, while staying connected to community, culture and the values that matter most.
