We are seeking a skilled RN Patient Care Manager to join our team in providing home health services.

In this role, you will supervise and coordinate clinical services, delivering high-quality nursing care to patients in their homes. Your expertise will help us achieve our goal of providing exceptional patient-centered care.

Job Description

The RN Patient Care Manager is responsible for:

  1. Providing clinical services within the scope of practice, as defined by state laws, in accordance with the plan of care, and in coordination with the healthcare team.
  2. Receiving referrals, assigning clinicians appropriately, evaluating patient orders, and initiating start of care visits.
  3. Coordinating determination of patient home health benefits, medical necessity, and insurance approvals.
  4. Assessing patient needs continually and ensuring care is individualized, appropriate, and meets eligibility criteria and physician orders.
Required Skills and Qualifications
  • Current RN licensure in the state of practice.
  • Current CPR certification.
  • Valid driver's license, vehicle insurance, and access to reliable transportation.
