
Care Development Specialist

We are seeking a dedicated and experienced Care Development Specialist to support our growth in Far North Queensland. The ideal candidate will have a strong background in community care.
The role involves service development, coordination, and delivery, including client assessments and care planning. This includes:
- Care Assessments
- Government-compliant care management plans
- Holistic outcomes-driven care planning
- Operation of care planning and other management systems
- Personal care supports
- Quality assurance

Required Skills and QualificationsTo be successful, you will need:
- A minimum of Certificate IV Aged Care
- Diploma in Nursing with current registration (highly desired)
- Current First Aid & CPR certification
- Driver's License
Prior experience in the government funding environment for aged care and NDIS would be highly valued, as well as good understanding of the hospital discharge process.
Please note that this role requires flexibility, including some weekend support, which can be offset during the week.
