Health Worker Opportunity
Sunshine Coast Health Service has a unique opportunity for an Aboriginal and Torres Strait Islander Advanced Health Worker to join our multidisciplinary team. This role will provide culturally safe, clinically competent, and professional care to clients across the continuum of primary health care.
- Deliver evidence-based interventions in a culturally safe manner, including co-case management, care coordination, intake assessments, cultural aspects of assessment and treatment plans, direct therapeutic interventions, referrals, and cultural support.
- Undertake health programs and community care duties that may include but are not limited to women's and men's health, antenatal, child and youth health, older person's health care, nutrition, substance abuse, sexual abuse, mental health, clinics, health promotion, and education.
- Manage own work with limited supervision through the use of planning and time management, showing initiative and positive attitude.
To be successful in this role, you must have a practical understanding of Aboriginal and Torres Strait Islander inequities and health issues. You will also need to demonstrate a strong ability to work collaboratively as part of a multidisciplinary team and provide informed, evidence-based culturally appropriate care.
