Care Coordination Manager

The Care Coordination Manager plays a crucial role in ensuring high-quality care is provided to residents at all times.
As part of our team, you will support the Clinical Care Coordinator and Registered Nurse to supervise and coordinate care across the home. Your focus will be on ensuring individual resident needs are met within a supportive environment where they feel valued and respected.

Casual shifts are available with flexible rostering, allowing you to balance your work and personal life.
Key Responsibilities- Provide social and emotional support to residents, prioritizing holistic and person-centered care.
- Support the coordination of care needs across the home, including medication management, nutrition, and hydration.
- Lead and supervise care on shift, focusing on designated wings or areas.
- Perform medicine-related tasks in accordance with scope of qualification, state or territory legislation, and Calvary policies.
- Relevant qualifications: Diploma of Enrolled Nursing (APRHA Registration) or Certificate IV in Ageing Support (HLTHPS007 Administer & Monitor Medicines).
- High level of commitment, integrity, honesty, and trustworthiness.
- Effective communication skills, with the ability to interact with residents, colleagues, and other stakeholders.
- Working knowledge of WH&S and Infection Control.
- Current National police check.
