Key Role in Multidisciplinary Team

We seek a skilled clinician to work closely with patients facing complex physical, mental health, and social challenges.

The successful applicant will have tertiary qualifications in Social Work or Occupational Therapy and current membership with the Australian Association of Social Workers (AASW) or AHPRA registration as an Occupational Therapist. You will have demonstrated clinical skills in assessment, planning and evaluation of psychosocial interventions, including exercising independent professional judgment on routine matters.

Key responsibilities include working directly with a medically led treatment team to provide frontline services, conducting comprehensive assessments, co-designing care plans, and providing assertive follow-up. This may involve home visits, strengthening referral pathways across community services, and contributing to team operations and reviews.

Successful applicants will be passionate about harm minimisation, recovery-focused care, and making a real impact through their work.

As part of our multidisciplinary team, you will work collaboratively with other healthcare professionals to achieve positive outcomes for patients.
