**Job Overview**
The Registered Clinical Nurse will be responsible for supporting the Clinical Care Manager and multidisciplinary team to identify and analyse data on identified indicators and areas of risk.
Key responsibilities include:
- Reviewing processes and documentation to identify gaps or trends and implementing staff education and continuous quality improvement initiatives in consultation with the Clinical Care Manager and in line with best practice guidelines, organisational policies, and relevant legislation
- Identifying AN-ACC rates in correspondence to resident care needs
- SUPPORTING THE RESIDENT ON THE JOURNEY WHEN THEY JOIN US
- Ensuring assessments, care strategies, and ongoing documentation to provide evidence to support AN-ACC funding and track funding for each AN-ACC resident
- Providing advice and support to Residential Aged Care staff around funding evidence requirements, working alongside the teams assisting in the resident assessment process
**Requirements and Qualifications**
To be successful in this role, you will require:
- AHPRA Registration and a bachelor's degree in nursing
- Strong ability to identify and implement areas for improvement initiatives to increase compliance levels and client satisfaction
- Experience in leading and training Care and Clinical team members while creating an engaging and supportive work culture
- Proficiency using various systems, including online care management programs
- Fully vaccinated for Influenza and COVID-19
**Location and Working Hours**
This role will be based in Carlton, working Monday, Tuesday, Thursday, and Friday 8am to 4pm, with a total of 30 hours per week.
