Job Summary

We are seeking an experienced Community Support Coordinator to join our team. As a key member of our team, you will be responsible for delivering community social support programs that make a positive impact in the lives of others.

Key Responsibilities:

  • Facilitate and evaluate social support programs, including community and outing-based activities.
  • Provide client-centered support, assisting with daily living needs, communication, and overall well-being.
  • Organize client transport to and from venues, using personal vehicles and community buses as needed.
  • Coordinate meal preparation and service, ensuring safety and quality standards.
  • Participate in improvement initiatives, attending relevant training and team meetings.
  • Maintain a safe environment for clients and staff by adhering to safety policies and performing safety audits as required.

Requirements:

  • Certificate IV in Leisure and Health, Ageing Support, Community Services, or equivalent.
  • Experience working with elderly clients and individuals with disabilities in a community care setting.
  • Strong understanding of client-centered programming, CHSP, and HACC PYP programs.
  • Knowledge of Food Safety and Occupational Health & Safety standards.
  • Strong communication skills and ability to work collaboratively in a team.
  • Basic computer literacy in Microsoft Office applications.
  • Valid Victorian Driver's Licence, reliable personal vehicle (with comprehensive insurance), and ability to drive a Council's 12-seater bus.
  • Current First Aid and Food Handling Certificates (or willingness to obtain).
  • Current Police Check and Working with Children's Check.
  • Bilingual skills in a community language are highly desirable.
