Support Coordinator Role

This is a pivotal opportunity to collaborate with individuals who require assistance with navigating their NDIS plan and budget. As a Support Coordinator, you will work closely with participants to identify their unique support requirements and develop tailored solutions to achieve their personal goals.

Main Responsibilities:

  • Design and implement individualized support plans that focus on empowering participants to take control of their lives.
  • Facilitate connections between participants and various stakeholders to ensure seamless service delivery.
  • Assist participants in understanding their plan funding and its purpose, as well as identifying and developing their skills and confidence.
  • Collaborate with the participant's network to resolve points of crisis and develop capacity and resilience.
  • Provide coaching and guidance to participants through challenges and difficulties.

Requirements:

  • Minimum Diploma of Social Work or Allied Health Degree
  • Current knowledge of legislation and other regulatory requirements in areas of equality, disability services, and occupational health and safety
  • Working With Children Check
  • NDIS screening check
  • First aid certificate

Preferred Skills:

  • Excellent verbal and written communication skills
  • Strong time management and ability to prioritize
  • Excellent attention to detail with a resourceful 'can do attitude'
  • Ability to use Microsoft Office suite of programs and other computer systems
