Senior Acute Care Specialist

The Community Mental Health Clinician position is a permanent, full-time role that involves providing crisis assessments, interventions, counseling, and psychoeducation in acute care settings.
Key Responsibilities- Conduct crisis assessments and provide timely interventions to individuals in acute care settings.
- Coordinate access to psychosocial supports and community services tailored to individual needs.
- Monitor and respond to changes in consumer risk status, ensuring timely escalation and intervention.
- Foster collaboration with internal teams and external partners for seamless, coordinated care.
- Champion consumer advocacy and address personal needs, concerns, and preferences in compliance with the NSW Mental Health Act.
- Hold a degree in Social Work and be eligible for membership of the Australian Association of Social Workers (AASW).
- Have experience in community mental health, including acute and triage settings, risk assessment, and crisis intervention.
- Be committed to holistic, person-centred practice that includes families and carers as part of the care team.
- Build strong partnerships across services to deliver compassionate wraparound support for complex caseloads.
- Be highly organized, with excellent communication skills and confidence using electronic medical records.
