Care Leadership Role

We are seeking a skilled and compassionate Care Manager to join our team.

  • This is a leadership role that requires strong clinical expertise and the ability to motivate and support staff.

About the Role

The Care Manager will play a crucial role in delivering high-quality care to residents, working closely with the General Manager to manage and lead the delivery of person-centred care.

Key Responsibilities

  • Oversee daily clinical operations to ensure residents receive the best care available
  • Support and lead the care home clinical team, including coaching, mentoring, staff allocations, delegation, performance management, and education
  • Review clinical incidents, complaints, and changes, and provide solutions to reduce risks
  • Lead compliance with AN-ACC assessments
  • Manage person-centred care planning and evaluation for residents, including case conferencing with families and carers
  • Foster a strong team environment
  • Prepare and conduct clinical governance audits
  • Provide insights and innovative solutions for continuous clinical improvement
  • Oversee medication management according to resident needs and legislative requirements
  • Liaise with allied health, medical practitioners, and other services to provide optimal resident care

Requirements

  • Bachelor of Nursing with current APHRA registration
  • Postgraduate qualification in aged care highly regarded
  • Strong and effective leadership, communication, and interpersonal skills
  • A person-centred care mindset
  • A passion for nurturing a strong team environment
  • Ingrained passion and empathy for our residents
  • Proven track record in delivering quality clinical care within an aged care or acute setting
  • Excellent clinical assessment and triaging skills, as well as strong care planning capabilities
  • Proven and demonstrated experience in fostering a reporting culture balanced with effective investigative skills
  • Understanding of Resident Rights, Aged Care Accreditation Standards, and Outcomes
