Care Coordinator – Transport Services

As a transport coordinator, you will play a vital role in ensuring the smooth operation of our client transport services.
The organization is dedicated to improving the health and wellbeing of Aboriginal and Torres Strait Islander people by providing high-quality services that meet their unique needs.
You will work collaboratively with a team of health professionals to develop and implement effective strategies for achieving positive outcomes for clients.
Key responsibilities include:

- Coordinate transport services for clients to attend clinics and specialist services
- Assist with data management and maintenance
- Maintain confidentiality of all clients and visitors
Essential Skills and Experience
- Hold a valid driver's license
- Hold or be working towards a first aid certificate
- Have extensive knowledge of metropolitan Adelaide roads and public transport routes
Desirable Skills and Experience
- Hold or be willing to undertake training in patient transport procedures
- Experience working with Aboriginal and Torres Strait Islander communities and organizations
- Experience using electronic patient information systems
