We are seeking a dedicated and compassionate professional to provide support to individuals with disabilities, promoting independence in daily living skills, transportation, social activities, and personal care.
Key Responsibilities- Assist clients with daily living tasks, encouraging self-reliance and confidence.
- Maintain accurate records using online systems for efficient customer management.
- Develop and maintain professional relationships with clients, families, caregivers, and stakeholders.
- Create and implement personalized care plans tailored to individual needs.
- Certificate-level qualification in Disability or Community Services/Individual Support, or relevant experience in the human services industry.
- A valid NSW driver's license (C Class) with the ability to drive large vehicles.
- Fitness to perform physical requirements of the role, including lifting up to 10kg unassisted.
- Intermediate skills in Microsoft Office and common systems.
