The core mission of this role is to support individuals with disabilities in living their lives to the fullest. This position presents a unique opportunity to be part of a team dedicated to making a positive impact.

This job involves coordinating and implementing support services for NDIS participants based on their individual plans, using a person-centred approach. Key responsibilities include:

  • Strengthening clients' ability to manage supports and participate in their communities.
  • Regular monitoring of clients' NDIS plans to ensure services are delivered within allocated budgets.
  • Maintaining effective relationships between clients and mainstream service providers (housing, education, justice, health).

Required skills and experience include:

  • Previous experience in a support coordination role.
  • Demonstrated ability to work effectively with people with disabilities and mental health issues.
  • Excellent verbal communication skills and ability to engage with clients and their families.
