Job Summary

We are seeking an experienced and passionate Clinical Nurse Specialist to lead our case management team in providing high-quality care to complex clients. The successful candidate will have advanced clinical skills and experience in managing substance use issues.

About the Role

This is a full-time opportunity to work with a dedicated team in Tamworth Community Health Service. The Clinical Nurse Specialist will work closely with the multidisciplinary team to provide individualized, case-managed care aimed at maximizing health outcomes.

Key Responsibilities
  1. Lead the case management team in providing high-quality care to complex clients
  2. Develop and implement individualized care plans to maximize health outcomes
  3. Work closely with the multidisciplinary team to provide comprehensive care
  4. Provide education and training to staff on managing substance use issues

Requirements

To be successful in this role, you will need:

  • Current Registration with the Australian Health Practitioners Regulation Agency (AHPRA) as a Registered Nurse
  • Post-registration qualifications relevant to the area of specialty with at least 3 years post-registration experience within the clinical area of specialty
  • Eligible to drive in NSW and willing to travel for work purposes
