About this role

  • Building strong connections with clients and their families is a core part of the job.

The Care Coordinator will be responsible for conducting comprehensive client assessments and providing professional health and clinical care advice to home care workers.

Key Responsibilities:

  • Developing meaningful relationships with clients and their families through regular communication
  • Conducting thorough client assessments to identify needs and develop care plans
  • Providing expert health and clinical care advice to support the delivery of high-quality home care services
  • Collaborating with clients and their families to ensure their needs are met
  • Maintaining accurate and up-to-date client records

Requirements:

  • Formal tertiary qualification in nursing or a related field
  • Registration as a Registered Nurse with AHPRA
  • Proven knowledge and experience within the aged care sector
  • National police check and industry applicable licenses
  • Valid First Aid and CPR certificate
  • Minimum of 2 COVID vaccinations
