We're seeking a passionate and dedicated Care Coordinator to join our team in Sydney's Lower North Shore. As a Care Coordinator, you'll play a vital role in coordinating in-home care services for our clients, ensuring they receive the highest level of care and support.

Key Responsibilities
  • Conducting client care assessments and care planning
  • Supervising and managing care staff
  • Discussing and promoting Right at Home's services with potential clients and referral sources
Requirements
  • Registered Nurse
  • National Police Check
  • Current First Aid/CPR Certificate
  • Driver's License
Preferred Skills
  • Sound understanding of the principles of My Aged Care, Aged Care Reforms, and Consumer Directed Care
  • Significant experience and understanding in the Australian Aged Care or Disability sector
  • Mandarin/Cantonese and English speaking skills
  • Empathetic, passionate, and caring approach
  • Experience in assessing and planning care needs
  • Client-focused approach based on their goals, preferences, and needs
  • Experience in managing staff
  • Comprehensive assessment and case management experience
  • Experience in providing care and services to people from culturally and linguistically diverse backgrounds
  • Commitment to working within Right at Home's Mission, Vision, and Values
  • Capacity to take initiative and be innovative
  • Highly developed organizational, administrative, business, people, and financial management skills
  • Customer service experience
  • Vehicle with comprehensive insurance
