This is a challenging opportunity to join a values-driven team in providing case management support.

Key Responsibilities:
  • Provide assertive outreach services, information, referral, advocacy support and link participants to services enhancing their quality of life.
  • Conduct initial assessments to ensure program eligibility and ongoing assessments as required.
  • Develop early intervention strategies to engage participants and achieve both program and individual goals.
  • Maintain accurate records of participants' personal information through electronic data systems.
  • Establish community partnerships via networking and referrals.
  • Attend professional supervision and development opportunities.
Requirements:
  • A degree-level qualification in Social Work, Human Services or Behavioural Sciences OR Certificate level 4 or higher qualification in the community sector with at least two years' experience in case management.
  • Excellent written and verbal communication skills, strong analytical skills and a valid driver's licence.
  • A current Disability Worker Screening and Working with Children Blue Card are essential.
