Job Title: Community Health Care Specialist

We are seeking a skilled and compassionate nurse to deliver holistic care to older adults experiencing mental health challenges and chronic conditions.

Key Responsibilities:
  • Providing direct clinical nursing care to clients in their homes
  • Conducting clinical assessments, dressings, medication assessment, and catheter changing
  • Developing a comprehensive care plan in collaboration with the client, carer, and healthcare team
  • Liaising with all parties involved in the care of the client to maintain overall wellbeing
Requirements:
  • A genuine passion for providing high-quality care to the elderly
  • Current AHPRA registration following completion of a Bachelor of Nursing or similar
  • Minimum 3 years post-graduate nursing experience
  • Experience in clinical assessments
