Mental Health Community Care Position

The role of a Community Mental Health Care Specialist involves providing patient-centered, evidence-informed care to individuals with severe and enduring mental illnesses.
The specialist will assess, plan, implement, and evaluate nursing care in collaboration with consumers and the multidisciplinary healthcare team.
