Job Title: Sil House Coordinator

Key Responsibilities:
  • Manage a team of support workers in a service outlet or community setting to ensure the delivery of quality care;
  • Develop and implement person-centered care plans that meet the unique needs of each participant;
  • Collaborate with participants and relevant stakeholders to gather input for developing care plans;
  • Assist participants in identifying goals through a person-centered approach and work towards achieving their individual objectives;
  • Undertake rostering of support workers to ensure participants receive appropriate care;
  • Ensure support workers implement BSP plans that meet the needs of participants;
  • Develop and maintain functional relationships with participants, family members, and stakeholders to promote positive outcomes;
  • Ensure the CRM database is up-to-date with all relevant participant information;
  • Manage allocated resources to deliver services within budget;
  • Coordinate the intake of new participants into services and minimize vacancies;
  • Provide supervision for all support workers to ensure they have the necessary skills and knowledge to deliver quality care;
  • Manage day-to-day service provision and provide leadership in all aspects of the operation regarding the SIL program;
  • Provide a safe environment free of abuse, harm, and exploitation for people we support;
  • Develop responses to participant issues using initiative and judgment under broad direction from the Service Manager;
  • Commit to continuous improvement principles, research, education, and adherence to set quality standards;
  • Adhere to legislation and organisational reporting, policies, and procedures to ensure appropriate health, safety, and wellbeing in the workplace.

Required Skills and Qualifications:

  • Degree in Social Work, Psychology, or related field;

  • Strong communication and interpersonal skills;
