Senior Home Care Specialist

We are seeking a skilled Senior Care Professional to conduct initial client intake and screening assessments. The ideal candidate will develop and review individualised care plans with a client-centered approach.
Key Responsibilities- Conduct thorough initial screening assessments, developing care plans for personal self-care, general house cleaning, social support, respite, transport and shopping assistance.
- Manage client information in CRM & CMS ensuring all details and case notes are kept up to date.
- Engage in ongoing care discussions, planning, assessment, reviews, and evaluating goals and outcomes in consultation with the care team.
- Provide quality care based on clients' needs, supporting with daily activities.
- Support clients with community based recreational and social activities.
- Transportation and assistance with shopping and appointments.
- Certificate III Aged Care, Individual Support or an equivalent qualification.
- Current Australian Driver's Licence (Green Provisional minimum).
- Roadworthy & insured vehicle to use for work.
- Experience with technology, including smartphones, tablets, laptops etc.
- Confidence to satisfy relevant probity checks.
