Job Description

This is a challenging and rewarding role that offers the opportunity to make a meaningful impact in rural and regional communities. As a Community Health Nurse, you will be working across multiple sites in the Limestone Coast region, supporting clients in their homes and communities.

The role involves assessing patient needs, planning and coordinating services, and providing expert nursing care that improves lives. You will work in a flexible, multidisciplinary model, partnering with consumers, communities, and fellow health professionals to deliver compassionate care where it's needed most.

You will have opportunities to extend your skills in the Immunisation Program, Wound Care, and Continence services.

  • Assessing and planning patient care to ensure the highest standards.
  • Collaborating with other healthcare professionals and community organisations.
  • Implementing and coordinating services tailored to each patient's needs.
  • Using problem-solving, negotiation, and communication skills to drive positive health outcomes.
  • Working with diverse groups, including Aboriginal communities, in a culturally sensitive manner.
Required Skills and Qualifications
  • Registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate.
  • Ideal experience in community health.
  • Ability to work within a multidisciplinary team.
  • Engagement with patients in a culturally appropriate way is a must, and a flexible, innovative approach to problem-solving is required.
  • A desire to work towards becoming an Authorised Immunisation Nurse.
