Job Title: Disability Support Worker
This role involves providing support to individuals with disabilities in a variety of settings, including their homes and community-based locations.
- Key Responsibilities:
- Perform direct care duties for individual clients as specified in the Personal Care Profile.
- Prepare meals and assist clients according to their Medical Treatment Management Plans (MTMPs).
- Provide additional support where required, such as subcutaneous injections, gastronomy feeding, etc., where appropriate training will be provided.
- Assist with clients' physical and mobility requirements in accordance with training provided and the relevant workplace health and safety standard.
- Maintain accurate client records and work collaboratively with clients, family members, and other stakeholders.
- Respond to crises within specified guidelines and seek assistance when necessary.
