Home Health Clinical Lead

Bayada Home Health Care is seeking a skilled Rehabilitation Manager to lead our home health team in the Shire of Towong.
The ideal candidate will have prior experience in Medicare Home Health and a strong background in physical therapy or occupational therapy. As a Rehabilitation Manager, you will oversee the delivery of high-quality client service and clinical care, ensuring that our clients receive exceptional support in their own homes.
- Main Responsibilities:
- Direct and coordinate care provided by a multi-disciplinary team.
- Lead field staff in delivering exceptional client service and quality clinical care.
- Ensure adherence to policies and procedures.
- Review clinical documentation, including OASIS and care plan coding.
- Monitor episodic utilization.
- Act as internal case manager and client advocate.
Required Qualifications:
- A current license in good standing, Physical Therapist (PT) or Occupational Therapist (OT).
- Prior supervisory experience.
- A minimum of two (2) years community health experience.
- Prior clinical and managerial experience within a Medicare Certified Home Health agency.
