Job Title: Aboriginal Health Worker

We are seeking a compassionate and culturally responsive health worker to provide high-quality care coordination services to Aboriginal families with children aged 0-5.

About the Role:
  • Deliver culturally sensitive nursing care to support the health and wellbeing of Aboriginal families.
  • Conduct home visits and offer practical health support based on primary health care principles.
  • Collaborate with multidisciplinary teams to develop personalized care plans that cater to the unique needs of each family.

Key Responsibilities:
  • Provision of culturally responsive care coordination services to Aboriginal families.
  • Collaboration with Child and Family Health Nurses and other allied health professionals to provide assessment, care coordination, and education to families.
  • Taking into account clinical, psychosocial, and cultural needs when delivering care.
Requirements:
  • Identification as being Aboriginal and being accepted in the community as such.
  • Certificate III in Aboriginal & Torres Strait Islander Primary Health Care or relevant qualification OR willingness to complete further training.
  • Demonstrated experience delivering culturally responsive health services to Aboriginal communities.
  • Demonstrated experience working effectively as part of a multidisciplinary team to provide care coordination or case management services.
  • Australian C-Class driver's license (including P1 or P2).
