About the RoleAs an Aged Care Assessment Team (ACAT) professional, your primary responsibility will be to conduct comprehensive assessments of clients' needs, determining eligibility for aged care services under the Aged Care Act (1997). You will work collaboratively with healthcare professionals and stakeholders to develop and implement personalized care plans, promoting independence and quality of life for older adults.
This is a challenging yet rewarding role that requires strong clinical expertise, excellent communication skills, and the ability to work independently and as part of a dynamic team. The successful candidate will possess relevant tertiary qualifications, full registration with AHPRA or eligibility for membership with the Australian Association of Social Work, and significant experience working in aged care settings. You will also demonstrate a proven ability to work flexibly across various ACAT sites and contribute to the development of effective care plans.
