About the Role

As an Aged Care Assessment Team (ACAT) professional, your primary responsibility will be to conduct comprehensive assessments of clients' needs, determining eligibility for aged care services under the Aged Care Act (1997). You will work collaboratively with healthcare professionals and stakeholders to develop and implement personalized care plans, promoting independence and quality of life for older adults.

The successful candidate will possess relevant tertiary qualifications, full registration with AHPRA or eligibility for membership with the Australian Association of Social Work, and significant experience working in aged care settings. Key Responsibilities
  • Conduct comprehensive assessments of clients' needs and determine eligibility for aged care services under the Aged Care Act (1997)
  • Develop and implement personalized care plans in collaboration with healthcare professionals and stakeholders
  • Work collaboratively with multidisciplinary teams to ensure seamless care transitions and outcomes
  • Provide education and support to clients, carers, and families on aged care options and services
