Advanced Nurse Practitioner Job Summary

The role of the Advanced Nurse Practitioner involves delivering high-quality care to patients in Primary Care settings.

Main Responsibilities
  1. To assess patient needs and provide telephone triage, determining whether face-to-face appointments are required.
  2. To work autonomously as a clinician, responsible for assessing, treating, and referring patients to specialist services when necessary.
  3. To adhere to Professional Conduct guidelines and national standards.
  4. To gather comprehensive patient histories, conduct physical examinations, and identify potential diagnoses.
  5. To refer patients for investigations or specialist services when appropriate.
  6. To make independent treatment decisions, including prescribing medications when necessary.
  7. To coordinate onward referrals to other healthcare services.
  8. To ensure continuity of care for patients.
  9. To work effectively both independently and as part of a multidisciplinary team.
  10. To lead the team by promoting best practice in patient care and treatment.
