Job Description

Job Title: Homelessness Outreach Coordinator

Job Summary:

Centacare North Queensland is seeking a highly skilled and compassionate Homelessness Outreach Coordinator to join our team. As a key member of our homelessness services team, you will play a critical role in providing assertive outreach support to individuals and families who are homeless or at risk of becoming homeless.

Key Responsibilities:

  • Provide case management support to individuals and families who are homeless or at risk of becoming homeless, including provision of information, referral, advocacy, and support to enhance their quality of life and status in the community.
  • Develop and implement early intervention strategies to engage participants and achieve both program and individual goals.
  • Conduct initial assessments to ensure program eligibility and ongoing assessments in line with work instructions, utilizing the Outcome Star tool to document participants' progress and barriers to achieving goals in support plans and at reviews.
  • Undertake outreach and community engagement activities as required, including provision of community education and support.
  • Utilize electronic data systems to record and maintain participants' personal information in accordance with service agreements, legislation, and program policies and procedures.
  • Comply with legislation, service agreements, and Quality Management System requirements.

Requirements:

  • A degree-level qualification in Social Work, Human Services, or Behavioural Sciences, or a Certificate level 4 or higher qualification in the community sector plus at least two years' experience working within a case management framework in a relevant field.
  • Excellent written and verbal communication skills, as well as strong analytical skills.
  • A current Driver's Licence.
  • Currently hold, and/or eligibility to obtain and maintain, both a Disability Worker Screening and a Working with Children Blue Card.
  • Desirable experience in group facilitation, experience working as part of a large and multi-disciplinary team, and willingness and ability to work outside normal office hours as determined by the Manager of Homelessness Services.
