**About this Program**

The **Young Carer Support Worker** position is an integral part of the delivery of the Carer Gateway services program. This newly created role is responsible for identifying and delivering appropriate support to Young Carers across South East Queensland.

The objective of the role is to undertaking intake, assessment and planning and referral/coordination services in an outreach setting for Young Carers who are unable to access services via the Contact Centre.

This role will identify and facilitate opportunities that promote the inclusion of, and build the capacity of, people and community groups to access Carer Gateway services and achieve healthy, strong and connected communities.

The new Carer Gateway Services have been created in the context of national reforms and represent an exciting new era for the delivery of innovative Carer services.

Carer Gateway Services consist of 6 unique service types:

- Carer Support Planning: Intake needs assessment and planning
- Carer Coaching
- In-person peer support
- In-person counselling
- Carer-directed support packages
- Emergency respite care

Reporting to the Coordinator Community Engagement and Volunteering the role will work under limited direction and be responsible for:

- Identifying Young Carers in the community and raising awareness about their caring role.
- Providing information, referral and support to Young Carers.
- Promote awareness of Carer Gateway supports to service providers including through the education system and other youth focussed organisations
- Support the delivery of specific projects and activities. This may include identifying potential partners, coordinating events and working with schools to identify and support Young Carers.
- Support Young Carers with a high level of customer service ensuring Young Carers are listened to with patience and non-judgement
- Provide Intake, Planning and Assessment services to Young Carers as outlined in the Carer Support Framework
- Work with Young Carers to establish and implement agreed action plans
- Ensuring the service approach incorporates the following service principles: Carer Focussed, Practical and Flexible, Inclusive
- Creating an environment which Young Carers will find welcoming, real and inclusive
- Develop supportive relationships with Young Carers which are positive and are built on Wellways values

**To be successful in this role**

Having lived experience of mental health challenges, you will share your lived experiences of mental health challenges to inspire hope and encouragement in the recovery journey of young people accessing YSAS BounceBack services.

You will also need:

- Minimum Cert IV in Youth Work, Community Services, or similar
- Lived experience of Mental Health Recovery
- Current valid Driver's License and the ability to undertake travel for the role
- Current Working with Children Check (Blue Card) and NDIS Workers Screening Check
- Right to Work within Australia
- Satisfactory National Police Records Check (less than 12 months old)
- NDIS Worker Orientation Module Certificate
- Evidence of COVID vaccination with booster or valid medical exemption
