Job Overview

We are seeking a highly skilled and compassionate Case Worker to join our team. As a key member of our Family Connect and Support Service, you will play a vital role in bringing together families, support services, and community resources to prevent crises from escalating.

Key Responsibilities
  • Conduct thorough assessments of family strengths and needs to identify main goals and develop effective referral pathways.
  • Make informed referrals based on identified goals and follow up with services to ensure successful engagement.
  • Assess and manage child protection risks for families through meticulous documentation and timely intervention.
  • Establish and maintain strong relationships with key stakeholders in the community to provide comprehensive support.
  • Accurately document all client interactions and related tasks to ensure seamless service delivery.
