We are seeking a highly skilled and compassionate Home Care Professional to join our team. This is a permanent full-time opportunity based at our location.
- A formal tertiary qualification such as a Bachelor of Nursing or equivalent
- Current registration with the relevant health practitioner regulation agency as a Registered Nurse
- A National Police Certificate
- A current First Aid and CPR certificate
- A valid Blue Card
Experience in the aged care industry, particularly in community home care, is highly desirable. You should also have strong communication skills and be easily adaptable to new software platforms.
As a Home Care Professional, your key responsibilities will include:
- Building meaningful connections with clients and their families
- Conducting comprehensive client assessments and developing care plans to support our clients' needs
- Providing clinical guidance and support to a team of caregivers
- Regularly communicating with clients and their families to address any concerns or satisfaction issues
- Maintaining accurate and up-to-date client records
