About the Role
We are seeking a dedicated and compassionate Case Manager to join our team at CatholicCare Diocese of Broken Bay. As a permanent full-time position, this role will provide high-quality case management to promote the welfare of children and young people in our care.
This role exists within our Intensive Therapeutic Care (ITC) and Therapeutic Supported Independent Living (TSIL) program in Sydney, which focuses on recovery from trauma, achieving positive life outcomes, and providing a range of placement options for children and young people.
As a Case Manager, you may work across various locations in Sydney, depending on need. Flexibility to work additional hours, including weekends, and undertake on-call duties is required for this role.

Main Responsibilities:
- Developing holistic assessments and case plans to support the safety, well-being, and permanency needs of young people;
- Working closely with young people, families, and stakeholders to develop pathways for transition through care and ultimately achieve independence;
- Timely completion of case notes, panel reports, and case work reports;
- Fostering positive relationships with young people, families, and stakeholders;
- Making referrals for aftercare, leaving care, and TSIL/SIL services.
Requirements:
- A degree qualification in Social Work, Psychology, or equivalent, with additional training or qualifications in developmental trauma, child protection, client intervention, and case management;
- Experience working with children and young people who have experienced complex trauma;
- Understanding of the Intensive Therapeutic Care model;
- An unrestricted driver's license;
- A Working With Children Check and Criminal History Check are required for this role.
Salary and Benefits:
The estimated salary for this role is $49.28 per hour, plus superannuation and salary packaging options. CatholicCare offers a focus on employee well-being, ongoing reflective supervision opportunities, access to training and learning opportunities, and an Employee Assistance Program.
