Professional Care Specialist

We are seeking a skilled professional to provide comprehensive support and bereavement services to family members and caregivers of patients in our Palliative Care Service.
The successful candidate will work closely with clients to deliver high-quality care, ensuring that their needs are met within a professional, organisational, legal and ethical framework.
Main Responsibilities- Provide coordinated support and bereavement follow-up to family members/caregivers of patients in the Palliative Care Service.
- Ensure consistent provision of information and psycho-education for bereaved family members/carers of patients in the Palliative Care Service.
- Foster awareness among bereaved family members/carers of available support services and resources.
- Deliver client/patient care that adheres to professional, organisational, legal and ethical standards.
- Tertiary qualification in Social Work with eligibility for membership of the Australian Association of Social Workers or AHPRA registration as a Psychologist.
- Advanced clinical knowledge, skills and experience in Social Work/Psychology, with an emphasis on evidence-based practice.
- Well-developed time management skills, written and verbal communication skills, and IT skills, including ability or willingness to use telehealth as a mode of delivery for clinical care.
