About the Role

We are seeking a highly skilled and compassionate Support Worker Multi Skilled to join our team at Chorus. As a Support Worker, you will play a vital role in supporting seniors and NDIS customers to remain in their homes, providing tailored person-centered services and activities of their choice.

Key Responsibilities
  • Provide support to seniors and NDIS customers with varying needs, including domestic assistance and community access.
  • Work collaboratively with customers, their families, carers, community agencies, and other professionals to ensure comprehensive support.
  • Develop and maintain strong relationships with customers, understanding their unique needs and preferences.
  • Contribute to a safe and supportive environment, promoting the well-being and independence of customers.
  • Participate in ongoing training and professional development to enhance skills and knowledge.
Requirements
  • Certificate III Individual Support - Aged Care or Disability, or other relevant qualification.
  • Experience working with seniors, people on the mental health recovery journey, and those living with disability.
  • Excellent communication and interpersonal skills, with the ability to work with diverse groups.
  • Enthusiasm and engagement with customers, with a proactive approach to their safety and well-being.
  • Current Australian Drivers Licence with no restrictions, and a reliable and roadworthy motor vehicle for travel.
  • NDIS Worker Screening Check (or willingness to apply) and Comprehensive motor vehicle insurance and registration.
