Care Coordinator Job Description

As a Care Coordinator, you will play a vital role in delivering exceptional care to individuals of all ages.

We pride ourselves on providing person-centred support that empowers our clients to live independently and safely in their own homes.

The ideal candidate will possess in-depth knowledge of the Aged Care Quality Standards, Aged Care Legislation, and related principles. You will be responsible for delivering person-centred care coordination to consumers, assisting with day-to-day operations, and undertaking initial intake assessments.

Your expertise in community care operations within a healthcare setting is essential for this role. You must also have excellent communication skills, the ability to multitask, and prioritise workload effectively.

Key Responsibilities:

  • Possess in-depth knowledge and comprehension of the Aged Care Quality Standards, Aged Care Legislation, and related principles.
  • Exhibit a strong commitment to providing person-centred support to clients.
  • Possess knowledge of the Aged Care sector, including Home Care Packages (HCP) and Commonwealth Home Support Programme (CHSP).
  • Deliver person-centred care coordination to consumers and assist in the day-to-day operations of the organisation.
  • Undertake initial intake assessments.
  • Maintain accurate administration, including updating reports and government portals.
  • Ensure compliance with relevant regulations and best practices.
  • Perform additional responsibilities as outlined in the position description.
  • Familiarity with the NDIS Quality Standards is desirable.

Requirements:

  • Diploma or Cert IV qualification in Health, Community Services, Welfare, or Social Science.
  • Working with Children Check
  • Current driver's license and access to a comprehensively insured vehicle.

  • Ability to multitask and prioritise workload effectively with effective communication skills.
