About the Role

We are seeking an experienced and compassionate Independent Living Support Professional to join our team in Wollongong and the Sutherland Shire. As a key member of our community-based care team, you will play a vital role in supporting elderly individuals to live independently and with dignity in their own homes.

Your primary responsibility will be to manage a caseload of community-based elderly individuals with Home Care Packages, assessing their needs, developing personalised care plans, and managing budgets in accordance with program guidelines. You will also onboard new customers and proactively manage existing ones with a person-centred approach, collaborating with our Clinical, Scheduling, and Service Delivery teams to deliver comprehensive care.

This permanent full-time role offers flexibility, allowing you to manage your own schedule each week while working autonomously from home and within the community. Regular visits to customers in their homes across Wollongong and the Sutherland Shire are required.

About You
  • Previous experience in community-based case management is highly desirable.
  • Experience with Home Care Packages is highly desirable.
  • You must have a current driver's licence and a reliable vehicle with minimum Third Party insurance.
  • Strong communication skills, both verbal and written, are essential.
  • Demonstrated empathy and compassion are necessary for this role.
  • Excellent problem-solving abilities and critical thinking skills are required.
  • Good organisational and time management skills are necessary.
  • Cultural competence and the ability to work with diverse populations are essential.
  • Advocacy skills to ensure clients' needs are met are crucial.
  • Collaboration and teamwork skills to work effectively with interdisciplinary teams are necessary.

An estimated salary for this role is $65,000 - $85,000 per annum, depending on experience and qualifications.
